Its central body is composed of an octagonal tower that ends in a dome, its altar is covered in gold leaf where it highlights the image of the Lord of Anguish, patron of Otavalo. Also within this temple is the Museum of Sacred Art, which contains historical and religious material from the 17th century onwards.
It is also known as the parent Church of Otavalo. Initially it was a small chapel. Between 1676 and 1679 it was transformed into a temple.
